As a result of our Engaged Plan efforts, we are making strides towards becoming a healthier workforce. Our Wellness Activities, paired with supportive resources, have helped us to decrease our rate of high blood pressure, reduce our tobacco rate and encouraged us to stay up-to-date with preventive screenings. This is great news, as the health of you and your family is AtlantiCare’s utmost priority. Regardless of your medical plan choice, you have the opportunity to complete Wellness Activities to earnWellness Credits. Engaged Plan: The Horizon Engaged Plan rewards you through “Wellness Credits”for the activities you participate in. The Wellness Credit value of any activities on the 2024 Activity Checklist that you (or your spouse/partner) complete, will reduce your deductible in 2025. PPO Plan: The value (up to $750 per employee and an additional $750 for covered spouse/partner) of any activities completed in 2024 will be subtracted from your 2025 per pay contributions divided over 26 pay periods. Waived: Employees who choose to waive their benefits are eligible to earn up to $750 inWellness Credits in 2024 to be applied to your HRA in 2025.

Online Health Assessment - $ 150

The Online Health Assessment is a brief, confidential survey that will be available to employees and spouses/partners from January 2024 through November 30, 2024. The Online Health Assessment can be completed by visiting https://myatlanticare.org . This year the amount you earn for your Annual Preventive Care Visit is determined based on the date of completion. If you complete your visit by April 30, 2024, you will earn $900 in wellness credits. If you complete your visit by July 31, 2024, you will earn $750 in wellness credits, and if you complete your visit by November 30, 2024, you will earn $600 in wellness credits. A $150 wellness credit is automatically provided to members who complete their Annual Preventive Care Visit with a Primary Care Plus provider. To find a Primary Care Plus provider near you, or verify if your current primary care provider is a Primary Care Plus provider, call 1-888-569-1000 . Please be sure to indicate you are looking for a Primary Care Plus provider. These options provide participants with additional opportunities to earn credits. • Wellness Seminars – $50 Each (max 2). Attend Health Engagement wellness education sessions. For class schedule visit https://myatlanticare.org • Complete your Annual Preventive Care Visit with an AtlantiCare Virtualist and earn $50 towards the Make it Personal section of the checklist. • LifeCenter – LifeCenter members who check in 80 times or more between January 1, 2024, and November 30, 2024, can earn $100 in wellness credits.

KnowYour Numbers The following wellness activities are part of KnowYour Numbers. You can earn partial credit for completing any of the KnowYour Numbers activities. Please know that it is your responsibility to submit and confirm receipt of theWellness Activity Certification Form on page 7 to Health Engagement no later than November 30, 2024, in order to receive the appropriate wellness credit. Description

Blood Pressure - $300

In 2023, if you certified with a blood pressure that was greater than or equal to 135/85 (either diastolic or systolic), you are asked to: • Lower your blood pressure below 135/85. KnowYour Numbers information should be submitted by completing the 2023 Wellness Activity Certification Form at the time of your Annual Preventive Care Visit. In 2023, if you certified with a BMI that was greater than or equal to 30, you are asked to: • Reduce your total body weight by 5% or BMI to below 30. KnowYour Numbers information should be submitted by completing the 2024 Wellness Activity Certification Form at the time of your Annual Preventive Care Visit. In 2023, if you certified on your Wellness Activity Certification Form that you were a tobacco user, you are asked to: • Complete a tobacco cessation class led by an AtlantiCare Wellness Specialist. Tobacco Cessation classes can be scheduled by calling 609-677-7507 . At your annual preventive care visit with your primary care provider, your provider will have the opportunity to indicate whether or not you have had a lipid/cholesterol screening within the last 5 years. If the provider indicates on your Wellness Activity Certification Form that you have met this recommendation, provides the date of the last screening, your total cholesterol and HDL, you will be provided with a $200 wellness credit once your Wellness Activity Certification Form is submitted to Health Engagement.

Preventive Screening Recommendations The right preventive care at every stage of life helps you stay healthy, avoid or delay the onset of disease, and keep conditions from becoming worse. See Preventive Screening Recommendations on pages 11-12 for a complete list. Wellness FAQs When am I eligible to have my next Annual Preventive CareVisit? You are eligible to have one, fully covered Annual Preventive Care Visit each calendar year. For instance, if you had your last Annual Preventive Care Visit in August of 2023, you are eligible to have your next Annual Preventive Care Visit any time after January 1, 2024. You do not need to wait the full 365 days between appointments. This year you will be eligible to earn “early bird” credits for completing the Annual Preventive Care Visit earlier in the year. Visits completed by: *The maximumwellness credits an Engaged Plan or PPOmember can earn remains $2,000 each. Only my systolic (top number) or diastolic (bottom number) blood pressure was greater than or equal to 135/85 when I certifiedmy KnowYour Numbers in 2023. Do I need to make a blood pressure improvement to earn the KnowYour Numbers credit? Yes. Even if one of the two numbers (your diastolic and systolic pressures) registers as greater than or equal to 135/85, employees and spouses/ partners are asked to take the appropriate steps in order to lower their blood pressure to earn credit. 4/30/24 are worth $900 7/31/24 are worth $750 11/30/24 are worth $600

Do I need to improve my body mass index (BMI) score by the end of 2024? If you certified with a BMI greater than or equal to 30 in 2023, you are asked to reduce your total body weight by 5% in order to earn the KnowYour Numbers credit. To calculate 5% weight loss, please use the following formula: 2023 CertifiedWeight x .05 = # needed to lose goal weight to achieve credit. Can I apply any extra wellness credits I earn to my spouse’s wellness credit amount? No. The wellness credits you earn will only apply to you. What resources are available to helpme improve my blood pressure and/or BMI? AtlantiCare Health Engagement is dedicated to connecting you with resources to help you make improvements in your biometrics. Resources such as the LifeCenter, engaged weight management program, registered dietitian, Livongo and telephonic coaching are just a few of the many resources available to you and your family. How can I check the status of my wellness activities? There is a drop-down box at the top of the page where members can select the year they wish to view. What if my results are submitted past the deadline? We adhere strongly to all deadlines established for this program. We recommend you keep track of your progress by viewing your Wellness Activity Tracker and confirming all relevant information has been received by the deadline. No grace periods will be allowed. It is your responsibility to ensure all documents and activities are completed by the deadline.

AtlantiCare’s retirement benefits partner offers monthly webinars on a variety of financial wellness topics including Buying/Selling a Home, Creating a Budget, the Basics of Savings & Investing, and much more. The AtlantiCare Down Payment Assistance (DPA) Program is a partnership between AtlantiCare and Address Yourself, an affordable homeownership mortgage program by New Jersey Community Capital, that provides down payment assistance grants up to $10,000 to AtantiCare employees seeking to become first-time homeowners in Atlantic City. To qualify for the program, employees must be in good standing, meet income guidelines, and utilize the purchased home as their primary residence. Short term housing support such as past due rent, first month's rent, security deposits, late payments, & furniture available for qualifying individuals. SFunds are issued on a Families First Electronic Benefits Transfer (EBT) card that works like a debit card. This card can be used in person and online at most grocery stores and some participating farmers markets. In need of grocery assistance? Register to receive monthly food pantry packages of fresh fruits, vegetables, frozen foods and pantry staples. AtlantiCare employees may be eligible for discounted membership rate based on income and household size.
